You have the right to:

- Be treated politely and professionally.
- Receive care without discrimination.
- Have your privacy respected during care.
2. Information About Your Care
- Know what services we provide.
- Understand your care plan.
- Know how often services will be provided.
- Ask questions and receive clear answers.
3. Participation in Decisions
- Help create your care plan.
- Refuse services.
- Stop services at any time.
- Be informed of any changes in your care.
4. Privacy and Confidentiality
- Have your personal and health information kept private.
- Receive a copy of our Notice of Privacy Practices.
- Review and request copies of your records.
- Ask us to correct information you believe is wrong.
5. Freedom from Abuse
You have the right to be free from:
- Abuse
- Neglect
- Exploitation
- Harassment

Your Responsibilities as Our Client
To help us provide the best care, we ask that you:
1. Provide Accurate Information
- Share correct health history.
- Inform us of medications and allergies.
- Tell us about changes in your condition.
2. Follow the Agreed Care Plan
- Work with Care Professionals respectfully.
- Allow access to your home during scheduled visits.
- Notify us if you need to reschedule.
3. Maintain a Safe Environment
- Keep walkways clear and safe.
- Secure pets during visits.
- Inform us of any hazards in the home.
4. Respect Staff
- Treat Care Professionals and staff with courtesy.
- Avoid abusive or threatening behavior.
5. Financial Responsibility
- Pay for services as agreed in your Service Agreement.
- Notify us promptly of any billing concerns.
