    RN in-home assessment

    A registered nurse visits the home to understand the full picture. Safety, daily routines, health background, and family preferences. The visit is complimentary with no obligation to start services.

    What we review:

    • Home layout and safety considerations
    • Daily routines (mornings, meals, evenings, sleep)
    • Mobility, personal care needs, and cognitive status
    • Existing providers, medications, and family involvement
    • Cultural, language, and privacy preferences

    Deliverable: A written, non-medical care plan with home safety recommendations, recommended type and frequency of care, and clear care goals. If you don't move forward, you keep the plan at no charge.

    Care plan, Care Professional match, and start of care

    Once your RN has completed the assessment, we turn it into a concrete, easy-to-understand plan and introduce a carefully selected Care Professional.

    Care plan

    • We share a written care plan outlining tasks, frequency, and priorities.
    • You can ask questions or request adjustments before we begin.

    Care Professional match

    • We match based on skills, personality, language, and schedule.
    • Whenever possible, we introduce the Care Professional with the RN present.

    First week

    • Your Care Professional settles in, learns routines, and builds rapport.
    • Our team checks in to ensure everything is running smoothly.
